    The accreditation site report repeats fundamental factual errors and misconceptions about fire extinguishers and fireproof cabinets.

    2.07 Local and Municipal Laws Non-Compliance | “The non compliance is indicated based upon the out-of-date fire extinguishers”

    4.02 Access and Storage Further Development | “Some of the file cabinets were not fireproof.”

    4.04 Clinical Records Further Development | “The team was unable to verify that the filing cabinets were fireproof”

    9.08 Campus and Facilities Further Development | The Team found multiple file storage cabinets in both buildings that do not appear to be fireproof. Clinic has at least 2 extinguishers last inspected July 13, 2018. All others were inspected on July 16, 2021.

    An inspection date is not an expiration date and fire extinguishers have a 10-15 year life span. Criterion 9.08 does not require all file cabinets to be fireproof. It requires that responsibilities for campus fire protection be appropriately assigned as documented in the previously submitted lease agreement. The institution has six fire proof cabinets for student records. Not all cabinets must be fire-proof.

    Standards for student records and data must be updated to reflect common standards for management, archiving, security and access.

    The institution formally notes that this physical on campus site visit, under active public health restrictions, surrounded by empty classrooms and offices without computers, the accreditation site report finds delayed posting of elevator certification by the commercial landlord as non-compliance for a sublease tenant.
